Notice of Infringement – Claim

  1. An authorized signature of the copyright owner or representative;
  2. Identification of the copyrighted material claimed to be infringed;
  3. Location of the infringing material and information to help us locate it;
  4. Contact information of the complaining party;
  5. A statement confirming unauthorized material use;
  6. A statement of accuracy under penalty of perjury.

Counter Notification – Restoration of Material
